    MyHealthProfile is an ongoing record of my medical appointments, diagnoses, prescriptions, medical recommendations, and any other relevant data that is useful to tracking my medical history. It allows me to track my own medical history as well as have my records available to my doctors without having to request past records whenever I see a new doctor or specialist.

  • Should I be concerned about the security of MyHealthProfile

    MyHealthProfile utilizes the latest internet security protocols to meet the latest internet security standards. These features are regularly updated to meet ongoing internet security demands. Our security is of the same standards and demands used in internet banking and other secure log-in sites.

  • Is this service free?

    Yes. This service is part of your treatment plan provided by the US Department of Health to ensure that the best possible treatment is afforded you.

  • Where is my information coming from?

    Your doctors, nurses, care managers, or other medical partner will enter data on your health care each time you visit them. You can track who enters data and on what date by clicking on the button, See who viewed my information on the Tools tab. This way you can be certain that your medical information is current and is consistent with what you have been told by your medical professionals.

  • What if I forget my password?

    If you forget your password, simply click on the link, "forgot password", and a new one will be emailed to you. Once you have logged in with this new password, you will be asked to create a new password for future use.
